Since your Grand Design Momentum M Class 395MS has a Rhino bolt pattern and a 20k GVWR the Gen-Y gooseneck adapter you would want is the part # GY98GR.

Since you have a Lippert 1621HD pin box currently the correct Gen-Y 5th Wheel to Gooseneck Pin Box adapter would be the part #GY48GR that you referenced as the part #GY98GR fits a different type. It fits the Rhino Box.
