I have a Paradigm 395ds and I purchased a PullRite 4500 OE Puck Series 24K SuperGlide. What type of capture/locking plate do I need for it?

That greatly depends on what pin box you have on your Paradigm 395DS. For example, we have the MORryde Pin Box Adapter # MR57ZR as well as the Gen-Y Capture Plate # GY98FR - both of these accomplish the same goal but are for different fitments so they aren't interchangeable.
If you can confirm what pin box is on your 5th wheel trailer I can see if we have something that's compatible.
