The best option to add a hitch to your 2002 E-450 Ambulance is going to be the Draw-Tite Ultra Frame Service Body Trailer Hitch item # 41990-07. There are no custom fit hitches availabe. This hitch can be bolted on or welded to your Ford's frame. This hitch fits frames that are up to 44" wide. It is a class V hitch with a 2" receiver, that has a 16,000 lbs towing capacity.
I have linked the installation instructions below so you can see the requirements and decide whether or not this will work for you.
