Yes, the Curt 5th wheel brackets # C16420, will fit a 2004 Dodge Ram 3500 long bed. However this is only part of a 5th wheel installation kit. For a complete kit that includes the brackets, rails, and all of the hardware use # C16420-204. I have linked a video showing a typical installation and a link to the instructions for you.
