The head of the B&W Companion OEM 5th-Wheel Hitch item # BWRVK3300 is 13" in diameter. You will want to use the Reese Lube Plate 12" Diameter item # RP83001. This 12 inch diameter plate works great on higher capacity hitches like your B&W.
For your lube points I recommend the LubriMatic White Lithium Grease - 14 oz. Cartridge item # L11354.
If you need a grease gun you will want the Heavy Duty Grease Gun - Lever Action - Aluminum item # PTW54290.
