The Blue Ox High-Low Adapter # BX88241 is only designed for flat towing applications where there is no tongue weight since there's not downward pressure from a trailer tongue. If you need a dual hitch adapter for towing and carrying a hitch mounted accessory, your best bet would be something like the Curt Multipurpose Ball Mount # D210. This is rated for 750-lbs tongue weight towing and up to 500-lbs for carrying an accessory.
