Do the hitches come with a electrical plug-in on the hitch itself or does the wireing just come out to the hitch and hang loose at or near the ball?

Trailer wiring harnesses are separate from trailer hitches. There was a time that some hitches had a place to hold a 4-Way connector cut into the hitch but that practice has been occurring less and less. Some hitches do have a bracket that is welded to the hitch for a trailer connector but not all of them.
For a 2012 Nissan Xterra without the OEM towing package, the correct wiring harness is # C56225. Once installed you can use the little cover on the end of the 4Way to hook around the safety chain loop on the hitch.
Or, you can use a mounting bracket like # 18144 with short extension # 18140 or long extension # C57202. I have included some links on the wiring and mounting brackets for you.
