Question:

In the event of a flat tire.. does this hitch make it difficult too remove the spare or would I need to remove the hitch to remove the spare?

The presence of the hitch will prevent the spare tire from dropping straight downward while it is being lowered. While lowering the spare, you will need to push forward on it until it clears the hitch cross tube, at which point it will drop free.

We have the same situation on my wifes Kia Sedona van, and I can tell you from experience that it is not difficult to get the spare off of the vehicle. The procedure will just be a little different than you are used to.
