The Replacement Sidewind Gear Kit for Bulldog Jacks - 7,000 lbs # BD500156 is what is used for the Bulldog Round, Pipe-Mount Swivel Jack - Sidewind - 15" Lift - 5,000 lbs # BD198250 you asked from.
I can't find any info on your Omni 5,000 but I've attached a photo of the gear dimensions in that kit to assist for comparison.
