What are the dimensions between holes that is drilled? Not the size of the hole

The vertical distance between the mounting holes of the # 5991-SM are 1-1/8 inches. When you use this shim you only use either the left or the right set of holes. I attached a picture that shows the back side of the Lend a Hand # AM-800 mounting bracket that shows this.
