The information you provided unfortunately doesn't give me enough info to help you get the correct hubs yet. What I need is either the bearing number printed on the face of the inner and outer bearing or the number on the races also printed on the face of the race, but often more difficult to read.
The other option is to use a digital caliper like # PTW80157 and measuring the spindle diameters where the bearings ride as well as where the seal is. These measurements will also help me get you the correct replacement hubs for your 12 inch electric brake compatibility. I've attached a photo and article to assist.
