The Kodiak Disc Brake Kit # K2HRCM1337-9DAC is a 7k axle disc brake kit with a bolt pattern of 8 on 6-1/2 inch bolt pattern.
If you inner bearing is 25580 and your outer bearing is 14125A currently this setup would install on your trailers spindles. You would need two of these kits for both of your axles. This comes with bearings and grease seals.
For an actuator kit that includes brake lines, a breakaway system and an actuator you would want the part # CAR-HBA16-2.
I attached a help article on adding disc brakes to a trailer for you to check out as well.
