What is a five lug tire with an extra tip/ nipple called and where can I get them?

It sounds like you are talking about a conventional steel wheel. These wheels have a raised section, see picture I have included, that a baby moon hub cap, # AM90086, will fit onto.
There are different sizes available in the conventional steel wheels. To get the correct one I would need to know the bolt pattern, the width, and diameter you need. I have included a link that shows how to measure for bolt pattern.
