Can i leave extended or is it a must to twist under ?

The fully-assembled PortablePET Twistep Hitch Mounted Step # HE3052 that you referenced does have a storage position under the bumper to help prevent bumps and bruises to the shins. Since the anti-slip step pad surface measures a substantial 22-inches wide x 19-inches deep it is a handy option to store it out of the way when not in use. The linked video demonstrates.
However, if your application is such that the potential for bumps to the shins is not a concern you could leave the step extended.
