Hello Joseph, thanks for reaching out. The Curt Trailer Hitch Receiver # C13361 weighs approximately 50 lbs. I went ahead and took a walk out to out warehouse to get the measurements you requested. The hitch is 39-3/8" wide measured from the center of the mounting holes. The rearmost hole and the frontmost hole are 13-3/4" apart. From the rearmost hole to the farthest edge of the receiver tube is 11". I edited a picture to help illustrate.
The # C13361 hitch will fit a 2004 Ram 1500 without a factory hitch. Keep in mind it will not fit HemiSport Quad Cab or Ram Rumble Bee models.
Let me know if there is anything else I can do to help!
