I wish we could help with the axle but we don't have any straight axles in the dimensions you need nor do we have any leaf springs that are 58 inches long. Most of the current leaf spring dimensions are about half that so what you'll most likely have to do is install new hangers to use a modern size leaf spring like the # e44SR which is for a 5,000 lb rated axle and is 25-1/4 inches long from eye to eye.
For a tandem axle hanger kit you could use the part # AP233-H248 which comes with everything needed.
To find the axle you're going to need you'll need to find a local trailer shop who can order you in custom axles based on the dimensions you need.
