Will this work with my 2005 GMC Savana 1500 if I have a 3 inch suspension lift kit installed or will I need a different model?

The SumoSprings Solo Custom Helper Springs # SSR-204-40 will work for your 2005 GMC Savana Van as long as the lift kit didn't increase the distance between the frame of your vehicle and where it the SumoSpring is supposed to make contact with the lower part of your vehicle (around the axle).
If that distance was increased then I recommend going with the RoadActive Suspension instead because it installs on top of the leaf springs so it will function as normal. Here is the part that fits your 2005 Savana Van:
