What comes in the box when i buy the trailer hitch? Does it also include a wiring harness and ball? Will this hitch fit all 3rd generation 4runners or just the 98?

The Curt trailer hitch # C13087, comes with the hitch itself and all necessary hardware to install it. Wiring, ball mounts, balls, and other accessories are sold separately. This is because different vehicle could require different size balls and ball mounts based on the trailers they are towing.
This hitch fits 1996 to 2002 Toyota 4Runners, all models.
I also recommend etrailer ball mount kit # 989900. It comes with 2 ball mounts, 2 balls, a hitch pin with clip, and a storage bag. For trailer wiring you can use T-connector # 118341. It just plugs in line with the vehicle tail light wiring. i have linked instructions for the hitch and wiring for you.
