So I need to loosen and lower the exhaust system and remove the heat shield? Also cut it away to make it fit?

That is correct.

To install the Curt Trailer Hitch Receiver # C29UR on your 2018 Chrysler Pacifica Hybrid you will need to remove two the exhaust hangers from the rubber isolators, and support the exhaust to make sure it does not get damaged. Next you will remove the heat shield you mentioned, and will need to trim it according to the included diagram.

Once the hitch is in place, you will reinstall the heat shield, and the reinstall the exhaust into the rubber isolators. You will also need to trim a section of the underbody panel for the hitch to fit through as shown.
