What are the measurements for the first rail from the back of the bad on ford f250 2003 w. Extended cab

The correct placement will depend on which install kit you are using. I would recommend using a custom kit like the one offered by Reese, part # RP50082-58.
The installation instructions for the # RP50082-58 (see link) state that the rear of the back rail is 32-1/4 inches forward of the rear edge of the bed (not the tailgate). This will place the centerline of the hitch approximately 2-15/16 inches forward of the rear axle centerline. These dimensions can vary if you use a different kit.
