The Andersen Camper Wheel Chock and Leveler, part # AM3604, includes one leveler and one chalk.

You could use just one of these on a tandem axle trailer, however it's best have one set on each axle to ensure stability. They would be configured like the picture below. Just make sure the trailer is less than 30,000 pounds as that is the most these can handle.
