Hot Spot Mirror Recommendation for a 2001 Ford E-350

I have a 2001 Ford e-350 van. I have seen ambulances that attach a piece of metal to the back side of the mirror housing and bolt a round mirror to the metal. Can the same be done with this mirror?

The round convex mirrors on emergency vehicles are designed to be mounted in the way that you describe. The CIPA clamp-on mirror, # CM49504, is designed to clamp to a round mirror bracket on a factory mirror. I looked at some pictures of the mirrors on the 2001 Ford E-350 van and I do not believe this mirror will fit.
For your mirrors I recommend going with a basic stick on mirror like # 49302. It is inexpensive, cost-effective, and will provide you with more visibility.
