Yes, the Kat's Heaters Custom Fit Engine Block Heater part # KH11813 is a confirmed fit for the 4.0L V6 engine in your 2018 Toyota 4Runner SR5. According to Kat's, the engine block heater will need to be installed on the left hand side of the engine (Driver) closest to the firewall. Don't forget the Kat's Heaters Plug Cover for Power Cords with 3-Prong Plugs part # KH28405 to cover the plug while not in use.
I have attached a photo showing this location for you as well which is labeled "LR".
