Is there a tool that helps tighten the knobs?

No, there isn't a specific tool made to tighten the replacement knob # 853-5714 to the rest of the load stop assembly on the Thule/DeWalt Ladder Tilt 339. We still have the load stops though so I grabbed one (off of a ladder rack that uses the same load stops) to see how it operates.
Basically, you will place the T-bolt and washer in the channel on the carrier. Then slide the load stop on placing the hole for the knob over the threaded part of the earlier inserter bolt. Then place the knob on the threaded end of the bolt and tighten down.
The knob for the load stop draws it tight to the carrier so it works on a sandwiching sort of principle. As such the knobs can come off if you loosened them enough so only loosen them enough to slide the load stops and hand tighten them once in position.
