What you are looking for is an engine oil cooler with a sandwich adapter. This will leave the oil filter in its current location. Cooler # D15405 is an excellent choice. It does have a sandwich adapter and multiple thread and o-ring sizes. It looks like your 1991 Chevy K1500 with the 350 motor uses either a 2-3/4 inch or 3-1/2 inch diameter o-ring. This kit has both. And the truck also uses either a 18x1.5mm thread style (most likely) or a 13/16-16 inch thread style. The kit includes both.
So with this particular engine oil cooler all the bases are covered! I have linked the instructions and linked a help article on oil coolers for you.
