The etrailer Trailer Hitch Receiver - Custom Fit - Matte Black Finish - Class III - 2" # e98844 is a confirmed fit for all models of 2019 Toyota Highlanders that are equipped with the steel spare wheel and tire. If you have an aluminum spare wheel and tire then this hitch will not work due to the extra width of the aluminum wheel.
In the pictures you provided, you will need to remove the plastic covering over the spare wheel and tire to verify which one your vehicle has come with from factory. If you are unsure on how to tell the difference between the aluminum or steel spare wheel, using a magnet can help determine the type of metal. A magnet will not stick to aluminum.
