I do not have any information about what specific roof vent cover would have been used on your camper, but I can explain how you can figure it out.
The first thing you'll want to do is get a measurement of the outside dimension of the vent that extends through the camper roof. Then, you'll want to closely examine whatever's left of the existing vent cover or the roof vent itself for a manufacturer name or part number. Also, examine the hinge used to connect the cover to the vent. Is it a continuous hinge, or is there a number of tabs on it?
Once you know the outside dimension of the vent opening and the hinge style needed, you can compare that with the available replacements I've linked you to. If you're still having trouble, please reply with whatever information you've gathered and I'd be happy to assist further.
