Hello, does this item include the vehicle specific connector for my 2005 Chevy Avalanche LT? Curt Next Brake Controller Item number C86SV.

It sure does; both the brake controller and the wiring adapter for your 2005 Chevy Avalanche are included with the Curt TriFlex NEXT Trailer Brake Controller w/ Custom Harness # C86SV.
This will be right for your truck so long as you have the factory 7-way, as the adapter will plug right into the port under the dash on your Avalanche.
