Can you leave the extension on the camper sewer pipe all the time and will it come off going down the road or twist off when you try to twist the cap off in

You can leave the Valterra Clearview Extension for RV Waste Valve # T1021 installed during transit but most people remove any types of connections like this during travel to make sure it doesn't come off and start bouncing around, plus adding anything other than the standard sewer hose can keep it from fitting in a smaller storage compartment like a bumper.
You mentioned the extension coming off when the cap is removed and while that is a possibility, if you just keep pressure applied where the extension is installed I don't see that being an issue.
So basically it's really up to you as it won't really hurt anything to leave it installed.
