Good question, and the difference is what they fit; the Curt Trailer Hitch Receiver # C15800 is designed to fit cab and chassis frames of not only a Chevy, but also Ram trucks. Cab and chassis frames are much narrower than pickup frames, which is why they are different fits. So if your Silverado 3500 is a cab and chassis, you can use the Curt Trailer Hitch Receiver # C15800.
Otherwise, if your '21 Chevy Silverado 3500 is a standard pickup, the correct fit for a 2-1/2 inch hitch is the other one you asked about, the Curt Trailer Hitch Receiver # C96CR.
