Is the hook on the top completely removable? I have an suv and the four swings out over the bumper and the hook is just a bit too tall. I don’t need the hook at all just the three sizes of balls. Thank you!

Your best option would probably be the Convert-a-Ball Interchangeable Ball Set part # 105TB which is a ball you'd install on a ball mount and gives you ability to easily swap out the three different ball sizes of 1-7/8 inch, 2 inch and 2-5/16 inch.
The hook of the ball mount you mentioned is not removable.
