Question:
Would this tire carrier work for a 22.5 tire approximately 40 inches in diameter and about 12 inches wide held under my 2000 Holiday Rambler Endeavor 38CDS motorhome?

There isn't a specific tire size limitation for the Lippert Components RV Spare Tire Carrier with Winch - 71" Overall Width # LC159041, just a weight limitation of 150 lbs. The carrier isn't really designed for motorhomes as it would be quite difficult to install as it's designed for mounting to the frame of a trailer.
If you have the room on your Holiday Rambler I suppose it is possible but a much better and easier option for carrying your tire with a 22-12 inch wheel is a hitch-mounted carrier, the Roadmaster Hitch Mounted Spare Tire Carrier with 2" Receiver Opening # RM-195225.
I have added links below to video reviews for you to check out as well.
