when it says spare no larger than 20 inches is that total width or wheel diameter? does it mean my regular will no longer fit?

I've seen where it says that in a few answers, as well as the tech notes, and they are referring to the wheel diameter - not the actual tire diameter. The most common tire size found on the 2002 Ford F-150 is 255/70r16 which has a diameter of 30-1/10".
The Curt # C15303 has 32-3/8" between the mounting holes (measured center on center) and roughly 34" between the side mounting plates. I've attached a photo of the hitch with a full sized spare installed, so as long as you do not have larger than 33" spare tires, this hitch will work fine.
