Which Gooseneck hitch ball would you suggest for my 2016 Nissan Titan XD? Which safety chains will work with your suggested setup? Anything else that I need? Is this the lightest and safest set up?

In order to use the Demco Recon Gooseneck-to-5th Wheel Trailer Hitch Adapter part # DM8550045 on your 2016 Nissan Titan XD with the factory pucks, I recommend the Reese Elite Series Pop-In Ball Kit part # RP30137 because it is standard height (no rise) which is what you will need for the above to work for you.
The safety chains are not required but if you want to anyway then you could use the Fulton Safety Chain with 1/2" Clevis Hook part # FCHA0070324 which will fit the Reese safety chain loops. The Demco Recon Gooseneck-to-5th Wheel Trailer Hitch Adapter is the most lightweight setup we have for you and it is very secure if installed properly. You will however need to be mindful of your turning clearance with a fixed hitch and short bed truck combo.
