1. Are the light bulbs included with the light? 2. In the picture, it looked like there was a plastic box or C shaped piece of plastic on the back side of the light. What is this for?

Yes, the Omega RV Double Dome Light # 328-001-902XPW includes the two 921 incandescent bulbs. The rear of the light (please see the linked photo) shows the back of the included on-off switch and also the two pigtail wiring leads for making your 12V power hookup.
If you ever want to replace the standard bulbs with high-efficiency LEDs that will use less power you can do so using an item like # DI74ZR, which is sold as a pair. This LED emits a pleasant warm white light and uses only about 1/10th as much power.
