Is there a five flat wiring harness available, or do I have to use the four with an adapter?

All of the trailer wiring harnesses for the 2011 Acura MDX have 4-Way flat connectors. In order to have a functioning 5-Way you would first install a 4-Way such as # C56038, # 118424, or # HM11143275 (they all install the same way) and then use a 4- to 5-Way adapter, # 20036, like you said.
