On the Safety Chains for Ranch Hitch 5th-Wheel-to-Gooseneck Coupler Adapter, # AM3109, the one end will clamp around the shank of the adapter and the hooks on the other end of the chain should hook to the safety chain U-bolts in the bed of your truck. Sometimes the U-bolts lay flat on the truck bed on each side of the gooseneck ball and are spring loaded, so you might not see them. You have to pull the U-bolts up from the bed to attach the chain.
If you do not have the U-bolts on your truck I do have available a few choices depending which model hitch you have. I have the Curt Replacement Safety-Chain U-Bolt Kit for Curt Quick Goose 2 Gooseneck Hitch, # C113 and the Draw-Tite Replacement U-Bolt Safety Chain Kit for Remov-A-Ball Gooseneck Hitch, # 6308.
I am sending a video link which shows the safety chain hooks.
