Question:
Do these come as a pair, or do I need to purchase two chocks for a trailer with tandem axles 4 wheels total?

The Ultra-Fab Chock and Lock Wheel Stabilizer # UF21-001080 that you referenced is sold individually. You should only need one to effectively chock your wheels/lock the trailer since blocking one side of the trailer will prevent both axles from rolling.
