Since your 1991 Dodge W350 has a flat bed I recommend using a universal hitch that gets welded to the frame:
- Buyers Products Class 5 Service Body Hitch Receiver # 3371801050
This gives you a 2" hitch receiver and has 9" mounting plates that can be adjusted along the crosstube to fit varying frame widths. Since flat bed trucks have a different setup than a standard truck bed style it's better to go with a universal style like this than trying to make a standard hitch with fixed brackets fit.
