Is there an adapter available to mount the old style Thule square bar to the factory installed rack/crossbars on a 2018 Subaru Outback?

There is not an adapter to mount a Thule square bar to factory crossbars. You'll need to take off your crossbars to mount the square bars.
You can use the following setup for the Touring version of your Outback Wagon:
Crossbars - SquareBar Evo Crossbars, part # TH712200
Feet - Evo Flush Rail Feet, part # TH710601
Fit Kit - Fit Kit for Thule Evo Flush Roof Rack Feet, part # TH186047
If you don't have the Touring, you can use this setup:
Crossbars - SquareBar Evo Crossbars, part # TH712200
Feet - Rapid Podium Feet, part # TH460R
Fit Kit - Fit Kit for Thule Podium-Style Roof Rack Feet, part # THKIT4063
