For the light switch plates, we have the Single RV Switch Faceplate # 37212845 and the Double RV Switch Faceplate # 37212875 that are white. We have the Single RV Switch Faceplate # 37214045 and the Double RV Switch Faceplate # 37212885 that are black.
In regard to the hub cap covers, it looks like your 2004 Fleetwood Revolution 40 has 8 lug wheels on the rear according to my research. If your rear wheel bolt pattern is 8 on 275mm, then the Phoenix # PXQ822R will work. If your wheels are actually 10 lug, then the Phoenix # PXQH1002R will work for 10 on 225mm bolt pattern. The Phoenix # PXPQH1401RHC will work if your rear wheels are the 10 on 335mm bolt pattern. If you have a different bolt pattern, feel free to give me the exact bolt pattern of your rear wheels and I can help you find the replacement hub cover.
