Question:
I have an 2003 Silverado 1500 2wd extended cab. Im curious as to what the width from bolt hole to bolt hole is? I look under my truck and looks narrower than what your hitches look.

The bolt holes of the Curt # 13322 Class III hitch are 39.37 inches apart center to center for the set of holes closest to the rear bumper and 39.44 inches apart for the set closest to the front of the vehicle as shown in the provided photo. The hitch is a confirmed fit for a 2003 Chevy Silverado 1500.
I've linked to an install video showing the hitch being installed on a 2003 Chevy Silverado 1500.
