The Replacement Tarp part # A45161 is designed to fit the 2004 - 2006 Toyota Tundra Double Cab - 6 Foot Bed Access with the Access Lorado Soft, Roll-Up Tonneau Cover part # 834532006694.
Based off of the information you input this is not the correct part for your 2012 Toyota Tundra 6.5' Bed. That uses the Lorado Roll-Up Tonneau Cover by Access- Tundra Standard Box part # 834532008285. After speaking with Agri-Cover she informed me that all replacement parts are selected by serial number. So if you don't mind getting that off of the cover or rails for me I can find the exact cover you are looking for.
