Question:
HOW DO I DETERMINE THE SIZE? DO I MEASURE THE BOTTOM OR THE OVERALL TOP?

When replacing your floor vent you will just want to take a measurement of the opening that your current vent actually drops into. The JR Products Undampered RV Floor Register # 37202-28955 that you referenced is designed for a 4" x 8" opening, which is pretty standard.
I went out to our warehouse just to take more detailed measurements and found that the raised inner portion of the register, which is what would fit inside the opening, measures exactly 7-3/4 inches long and 3-3/4 inches wide. As long as your opening is at least this large it will fit and work well.
