I need to know if anyone makes a towing plate to fit a 2004 Honda Civic Hybrid Sedan?

Roadmaster and Blue Ox both offer base plates for the 2004 Honda Civic Hybrid Sedan and these can be seen on the link provided. Both feature twist-lock removable draw bars and both will require some trimming of the bumper fascia and other components. The Roadmaster kit is part # 521537-1 and the Blue Ox kit is part # BX2236.
The Roadmaster Tracker tow bar # RM-020 that you referenced is compatible with these Roadmaster base plates so I suggest these for your application.
Flat towing also requires a wiring/lighting solution, safety cables and a supplementary braking system for the toad. All of these items are detailed in the linked article on flat towing.
For safety cables I recommend the coiled type that will not drag on the ground, part # RM-643. For the easiest-to-install lighting kit you can use a set of magnetic lights like # TLL21RK. For a supplementary braking system, which is required for most states and provinces, you can use the portable Blue Ox Patriot, part # BLU37TR.
It is always a good idea to check you car's owner's manual for any required preparation to flat tow the vehicle.
