How do you loosen the star nut for the electric brake adjustment on a 12.25x5” 12k pound axle. Is there a keeper to keep it from loosening?

In order to loosen the star nut on electric brakes you can use Adjustment Spoon # W80630 you should be able to rotate the star counter-clockwise or "down" to loosen them.
There should not be any type of keeper that will keep it from loosening; if you take a look at the video I linked and forward to about the 4:25 mark you can see how the brakes should be adjusted.
