Hi Jerry!

The Reese Quick-Install Custom Bracket Kit for 5th Wheel Trailer Hitches 50081 (our part # RP50081) are a fit for a 2004 to 2014 Ford F150, in case you were curious. I was.

With that said, the rails to go along with them will be the Reese Universal Base Rails for 5th Wheel Trailer Hitches - 10 Bolt # RP58058. This includes the hardware needed to attach them to the frame brackets as well.

As to the sliding hitch, I recommend going with the B&W Patriot 5th Wheel Trailer Hitch w/ Slider - Dual Jaw - 18,000 lbs # BWRVK3270. This is technically overrated for your truck, but B&W makes the best stuff on the market. And they are American made!

What kind of trailer will you be pulling?

Looking at a few light weight, 9000 pound or less 5th wheels, they all have under 1000 pound hitch weight

@JerryT Makes sense. Do you already have a break controller? And, how if your suspension looking? A lightweight 5th wheel should mess with your suspension too much, but we do have options to beef it up if it looks like it could use some help when you are towing. : )

@LazerS I have the Max tow package on my F150, previous owner also installed airbags for stability. Not sure what they hauled but the brackets have 2 hols on the back and one hole on the front

@Jerry Air bags are good and was going to suggest them if your suspension was looking a little aged. As to the holes, that is normal and how it should be. If you go to the link I have included, you ca see in Figure 1 how the holes are laid out.
