How does using this system effect towing quality? Does it make towing a 5th wheel feel more unstable/increased sway?

The Gen-Y Hitch Gooseneck Trailer Hitch for GM OEM Puck Systems # GY73XR will not impact the ride quality even though it has a 5" offset. It will be the same as using a standard gooseneck ball since the weight is carried at the same location in the shank so you won't notice a difference.
