The question is Does the 12x4 hook come with the wiper set, or is there another Part I need to order? If so, what is the Part number?

The ClearPlus 77 Series HD Windshield Wiper Blade part # CP77261 which is a fit for a 2006 Fleetwood Expedition Motorhome on both the driver and passenger, comes with the adapter you need to install. There are three in the kit, the one you need to use is the one labeled for a 12x4 Hook.
