does this product come with the rails that are used for it to slide the rails that actually mount to the top of the truck bed?

Yes, the Thule TracRac SR Sliding Truck Bed Ladder Rack # TH43002XT-000 does include the black rails that mount into the stake pockets of your 2004 GMC Sierra bedrails and allow the towers to slide.
